I, as named within this form, agree to use the facilities operated by Finding The Lovely, LLC, dba Jenifer Warren Studio at 3342 Pearl Road, Suite 6, Medina, Ohio 44256, or participate in activities sponsored by Finding The Lovely, LLC, upon the express agreement, understanding, and condition that:

1. I acknowledge that I participate in activities, including but not limited to, the construction of glass art, the use of production equipment handling of raw glass materials (herein “activities”) that take place at the facilities operated at Jenifer Warren Studio.

   
   

2. I recognize and agree that I am voluntarily participating in these activities and using Finding The Lovely, LLC facilities, and that I assume all risks of injury, illness, and other damage or loss inherent in the handling of glass or glass fabrication by-products, such as skin cuts and abrasions.

   
   

3. In consideration of the agreement of Finding The Lovely, LLC to sponsor activities and/or provide facilities for my use, I am participating in such activities and using such facilities with the express agreement and understanding that I am waiving, releasing, and discharging Finding The Lovely, LLC and each of its Trustees, officers, employers,

   
   

   agents, and representatives from any and all claims, causes of action, costs, liabilities, expenses (known or unknown), (herein, collectively “claims”) for myself and my personal representatives, heirs, executors, administrators, agents, and assigns arising out of my participation in activities at the Finding The Lovely, LLC facilities or at activities sponsored by Finding The Lovely, LLC and any loss, damage, illness, or injury resulting therefrom.  I further agree to indemnify and hold harmless Finding The Lovely, LLC and each of its trustees, officers, employees, and representatives from and against all such claims.

   
   

4. I agree to follow all rules and regulations promulgated by Finding The Lovely, LLC for the use of the Finding The Lovely, LLC facilities or in connection with activities sponsored by Finding The Lovely, LLC.

   
   

I acknowledge that I have carefully read this Waiver and Release and fully understand that it is a release of liability.  I acknowledge that I am waiving any right I may have to bring legal action to assert a claim against Finding The Lovely, LLC dba Jenifer Warren Studio for negligence.
